Screen Shots of your Intake Forms

Hey Laurie - hope you are well.

I’ll post this to the 4 leg forum also - but wanted to ask you specifically. I’m still struggling to find a convenient fast way to take care of the paperwork end of rehab. I’m creating my own form, but am always reinventing the wheel. I know in the past you’ve said you use a software program tailored to animals - could I ask you basically to simply take some screen shots of your software so I can see what and how you are documenting? Always looking to perfect what I’m doing…

Hi Cheryl!

Okay… so documenting is one of the only things we aren’t yet doing online. I like paper!
But if I get a chance, I’ll go on to that side of things and see what it looks like and try to send a pic!
The company we use is called Practice Perfect… they might have a tutorial online or show pics on their website.
They’ve always told us that we can upload any form we’d like.

Sorry to not have that answer / ability at my fingertips!

And here is what the software would allow.
They will also allow you to upload your own forms so that you can customize your IA / Treatment charting / Discharge charting.

And please find attached the screen captures of the various intakes (we haven't uploaded our own yet.)

Ummm.... and we worked with them to customize all of the things we possibly could to make it 'animal' functional vs 'human'.
