Hi Wendy! Great question AND good case for discussion. I like that you are approaching this case with practicality. I've had a case like this just a couple of years ago. Honestly, I'm not going to claim any sort of ability to read or interpret imaging.
Everything you describe (signs/ symptoms) make me think LSDD.

I'm not sure how to fit IBD or loose stools into the picture however...
