Grade 3 Patellar Luxations

HI Laurie, 

 

I am searching for some experience that I do not have yet. 

 

I recently saw a 6 month old labor doodle pup who has grade 3 LPLs Bilaterally. The surgeon, who is usually very positive supporter of PT/ Rehab, but for this case scenario he is saying “no” to PT due to mal-alignment of the patellar quadricep mechanics, his thinking is that any strengthening/ stretching/ therapeutic ex, may make the pup worse. He is holding off of surgery until physes are closed at age 9 - 12 months of age. Owner wants to give PT a try of which I feel optimistic that maybe therapeutic exercise/ UWTM may help to strengthen this dog and provide some functional stability to the stifles. 

 

What has your experience been with such scenarios?

Okay, so as for your dog, unfortunately the grade 3's don't seem to be able to strengthen up enough to provide functional stability.  That patella just doesn't live in the groove enough of the time.  Grade 2's... absolutely fair game!!

 

Now, on the other hand, I don't think the surgeon's recommendation is appropriate either.  I would sell your services as 'pre-hab'.  In other words, working on SPECIFIC targeted exercises (Quads & Glutes) and incorporating some modalities for pain relief / reduction of inflammation could help the stifle, addressing spinal dysfunctions of myofascial trigger points could help normalize limb function... and all together this is more beneficial and less problematic than the dog running like a maniac at an off leash part with questionable stifles!
